Renewal Application: Non-electronic
Submitting your renewal via CORA is highly recommended for a faster, guided process. You can still do so by clicking here.
If you choose to submit a PDF application, processing will take longer and may require follow-ups from your underwriter. Be sure to submit before the deadline in your notification letter to avoid non-renewal.
Applications are reviewed in order of renewal date, and due to high volume, processing may take several weeks. Once reviewed and approved, you’ll receive your proposal and invoice to complete your renewal.

- Individual Policies: Download .pdf of Renewal Application
- Group Policies: Please select the state of your group practice below to download the group-specific entity .pdf. (Group policies: Your client ID is a 6-digit number starting with 60 (i.e., 60####))
